Semaglutide is the compound behind Ozempic and Wegovy — two names that became part of the broader conversation about weight and metabolic health in a way few medications ever have. It works by mimicking GLP-1, a gut hormone that tells your brain you have eaten, slows stomach emptying, and helps your body manage blood sugar. Originally developed for type 2 diabetes, it became one of the most studied medications for obesity after clinical trials showed weight loss results that exceeded anything previously approved in that category. It is available as a weekly injection or a daily pill.

Weight Management
The STEP trial series tested weekly injectable semaglutide (brand: Wegovy) in people with obesity. Average weight loss was around 15% of body weight over 68 weeks at the 2.4mg dose — more than had been seen with any previous approved weight medication.
Type 2 Diabetes
The SUSTAIN trial series tested semaglutide for blood sugar control. It is approved for type 2 diabetes in both injectable (Ozempic) and oral (Rybelsus) forms and reduces HbA1c significantly compared to placebo and many existing treatments.
Cardiovascular Outcomes
The SELECT trial enrolled over 17,000 people with overweight or obesity and established cardiovascular disease. Semaglutide reduced the risk of major cardiovascular events (heart attack, stroke, cardiovascular death) by 20% compared to placebo over about 3 years — a landmark result.
Kidney Disease
The FLOW trial found semaglutide significantly reduced the progression of chronic kidney disease in people with type 2 diabetes and CKD, adding kidney protection to its established cardiovascular benefits.

Long-term data beyond 3-5 years are still limited for most outcomes. The pill form of semaglutide is absorbed less consistently than the injectable — how much actually reaches the bloodstream depends on whether it is taken correctly on an empty stomach, which affects how reliably it works. GI side effects are common at initiation.

GLP-1 is released from the gut after eating and sends signals that tell the brain you have eaten enough, slow stomach emptying so food is processed gradually, and stimulate insulin release only when blood sugar is actually elevated. Semaglutide is an engineered version of GLP-1 with modifications that make it last much longer — about a week for the injectable form. By activating GLP-1 receptors continuously at a low level, it reduces baseline hunger, slows stomach emptying between meals, and improves the body's blood sugar regulation over time.

FDA-approved with a large safety dataset from years of trials and real-world use. The most common side effects are gastrointestinal — nausea, vomiting, constipation, diarrhea — and tend to be strongest when starting or stepping up the dose before settling down. It carries the FDA's most prominent safety label about thyroid C-cell tumors seen in rodent studies; the relevance to humans is not established, but it is avoided in people with certain thyroid cancer histories. Rare cases of pancreatitis and vision changes have been reported. Not recommended during pregnancy.
